All referrals are managed by Hospices of Birmingham and Solihull (HoBS). HoBS brings together specialists from Birmingham Hospice, Marie Curie West Midlands and Solihull Macmillan teams to provide responsive, expert support and advice for people and families needing effective symptom management, equipment to remain at home and arranging packages of care if needed, especially at end of life.
Referrals to services at Birmingham Hospice can be made using one form.
A new referral form for our HoBS service is now available.
Birmingham Hospice, Marie Curie, Solihull Macmillan and St Giles have worked together to develop an updated form to improve the patient journey. It was a collaborative decision by all of the HoBS providers to make this change.
We hope that this new form will make it clearer to our referrers what information is required when making a referral to ensure that patient is accepted, creating a more efficient service.

Healthcare professionals can refer patients to the following services:
- Community Palliative Care Services
- Hospice at Home
- Inpatient Unit
- Day Hospice and Living Well Centre services
- Therapies including occupational therapy, physiotherapy and Allied Health Professionals
- Breathlessness programme at Selly Park.
