This forms part of a series of interactive webinars, commissioned by Birmingham and Solihull ICB and provided by Birmingham Hospice in collaboration with Marie Curie and St Giles Hospice. Sessions are delivered by local palliative care experts.
Each live webinar is free to join and will enable you to share and learn from your own and other healthcare professionals’ experiences. The sessions will equip you with the tools and resources you need to confidently manage challenging situations that may arise when caring for patients with palliative care needs and their families.
As winter approaches and the demands upon the health and care systems increase, this webinar will provide attendees with an overview of the palliative care pathway and its importance for providing good care for patients at end of life. The session will explain the roles of the various services involved, how they integrate and the role of specialist palliative care providers. It will focus on person-centred care, admissions avoidance and how to facilitate timely discharge, to deliver better outcomes for patients, carers and families. It will be useful learning for those working in acute and specialist NHS trusts, Primary Care, the Ambulance service, local authorities and social care.
As part of our commitment to the NHS England comprehensive model of personalised care, these sessions will highlight the importance of shared decision-making and personalised care and support planning when working with patients and families.
