Patient and Carer Information Volunteer

                         

Reports To: Sharon Hudson- Community Development and Partnerships Lead

Location: Community Development Office

Hours: 5-8 per week - flexible but Mondays and Fridays preferred

Making a Difference

To work with the Family and Carer Support Team (FaCST) and the Community Development Lead to provide an effective information service for patients and carers. You will work alongside the existing Information and Support Role in the Family and Carer Support Team to extend the collation, distribution and updating of information for patients, families, carers and professionals to support information, enablement and onward referral.

Key Duties

 

 

 

You will…

Collate a database of patient and carer support groups and information through contacting community, health and voluntary sector partners

  • With the existing FACST volunteer, support the creation of an online database of accessible information for patients and families (with the support of the FACST team, PR and Marketing and the Community Development Lead)
  • Maintain this database to ensure it is up to date and relevant for our population
  • Disseminate this information to ensure it is available to patients, families and staff
