PERMANENT POST

37.5 HOURS PER WEEK – flexible working essential to cover a 7 day a week a service

HOSPICE BAND 3 - £15,654 - £19,140 per annum dependent upon experience. 

Car Owner/Driver Essential

Our Hospice at Home service provides personal, social and emotional support to patients and their families at home with life limiting illnesses. The aim of the service is to provide care at a time of identified need, and to help avoid unnecessary and unwanted admission to hospital in the last weeks and days of life. The team are a part of an integrated community team and there will be opportunities to work across all community services

We are seeking to recruit an emotionally intelligent and motivated HCA who will assist in supporting excellent end of life care.

The post holder will have a supporting role in providing support for patients with end of life care needs assisting in the delivery of care.

Successful candidates will have a NVQ Level 3 in health and social care and experience of working with people with palliative care needs. Community experience is desirable ( for full details see job description)
