INTEGRATED COMMUNITY PALLIATIVE CARE TEAM

2 posts; 37.5 and 22.5 hours

Hospice Pay Band 7: £32,172 - £42,414 (or Band 6 development £26,904 - £36,111), pro rata for part-time post.

Please note Hospice Band 6 Development to Band 7 post will be considered for an excellent candidate not meeting essential criteria for Band 7.

Do you have what it takes to drive forward community palliative care, supporting the Hospice vision for equal access to palliative care services for anyone with any condition wherever they live?

We are looking for an enthusiastic, highly skilled individual who is a team player, dynamic and passionate about palliative care and the role of the Clinical Nurse Specialist. The post involves working within a multi-disciplinary team offering specialist palliative care to patients in their own homes and our primary care clinics. Excellent leadership and communication skills are needed to fulfil this role. Teaching experience is essential in older to share knowledge and skills and enhance high standards of palliative care.

Band 7 Candidates will have a palliative care or other relevant degree and a recognised post registration qualification in palliative care (minimum of 20 credits at 1st degree level or working towards). In addition appropriate experience at NHS Band 7 (or equivalent) and clinical skills underpinned by advanced knowledge of palliative care and current issues is essential.

Band 6 Candidates will have a Palliative Care or other relevant degree and appropriate experience at Band 6 or above. A competency programme will be offered with the opportunity to complete the European Certificate in Palliative Care

A car owner and driver is essential.

The role includes on call responsibilities (telephone advisory service) and weekend work on a rota basis.
